Abstract
According to the World Health Organization(WHO), the maternal mortality rate in 2016 was estimated at 303.000 souls while developing countries at 239/100.000 live birth. In 2015, the maternal mortality rate in Indonesia was at 305/100.000 live birth. In 2017, the Riau islands' maternal mortality rate was at 127.96/100.000 live birth, while Batam city was at 105/100.000 live birth. Moreover, in 2015, the infant mortality in Indonesia was at 22.23/1000 of live birth. In 2017, the Riau islands' infant mortality rate was at 7.63/live birth, while Batam was at 5.7/live birth. The final project preparation aims to provide midwifery care of a pregnant mother, maternity, puerperal, newborn baby, and family planning continuously, and documented using the SOAP management approach. This project used interviews, observation, and physical inspection methods. The midwifery care in Sapta Tirta Clinic of Fitri Novi Palupi, Amd. Keb midwife in Continuity of Care way. Midwifery care was given to Mrs. M of 37-years-old for seven weeks. The frequency was from the pregnancy period conducted two visits. Then, in 37 weeks gestation, the mother has complained about the tightness in the solar plexus. The labor process occurred in the normal process at the Sapta Tirta Clinic of Fitri Novi Palupi Amd. Keb. In stage I, there was no assessment conducted because the patient arrived with a complete opening of labor. Then, in stage II was 10 minutes, and the baby boy was born at 00.15 WIB with 2500 grams of body weight and 47 cm of body height. In stage III was 5 minutes, and stage IV was 2 hours.
