Abstract

PROGRAM STUDI DIII REKAM MEDIS DAN INFORMASI KESEHATAN UNIVERSITAS AWAL BROS Karya Tulis Ilmiah (KTI), 2022 ANALISIS KETIDAKLENGKAPAN PENGISISAN BERKAS REKAM MEDIS RAWAT JALAN DI RUMAH SAKIT BHAYANGKARA BATAM TAHUN 2022 Puan Amanina Bey Husdi, 00419016 X+ 66 Halaman +2 Bagan + 25 Tabel + 8 Lampiran ABSTRAK Latar Belakang : Kelengkapan pengisian rekam medis penting dilakukan karena berfungsi untuk tanda bukti sah dan secara hukum dapat dipertanggung jawabkan. Cara untuk menilai kelengkapan dan keakuratan rekam medis dan menemukan kekurangan khusus pada pencatatan rekam medis pada rawat inap dan rawat jalan di sarana pelayanan kesehatan adalah dengan menganalisi kelengkapannya. Berdasarkan wawancara yang peneliti lakukan di Rumah Sakit Bhayangkara Batam indikator mutu yang diharapkan harus tercapai 100% namun berdasarkarkan data di tahun 2021 ketidaklengkapan berkas rekam medis mencapai 17,75% dan kelengkapan berkas rekam medis hanya 82,25%. Maka dari itu Kelengkapan pengisian berkas rekam medis harus mencapai angka 100% selama 1x24 jam setelah pasien keluar rumah sakit. Tujuan : Mengetahui Persentasi ketidaklengkapan pengisian berkas rekam medis rawat jalan Triwulan di Rumah Sakit Bhayangkara Batam. Metode : Metode penelitian yang dipakai di penelitian ini memakai metode Deskriptif kuantitatif dengan melakukan checklist dokumen berdasarkan tujuan penelitian. Hasil : Persentasi ketidaklengkapan identifikasi pasien pada lembar rekam medis di Rumah Sakit Bhayangkara Batam yakni 0%. Persentasi ketidaklengkapan pada pelaporan penting adalah 7,95%. Persentasi ketidaklengkapan pada autentifikasi adalah 8,8%. Persentasi ketidaklengkapan pada pendokumentasian yang benar adalah 2,75%. Persentasi ketidaklengkapan pada lembar rekam medis di Rumah Sakit Bhayangkara Batam yakni 5,1%. Kesimpulan : Ketidaklengkapan pengisian berkas rekam medis dikarenakan petugas tidak menerapkan SOP yang ada selain itu dikarenakan dokter yang terburu-buru dalam melakukan pengisian berkas rekam medis. The way to assess the completeness and accuracy of medical records and to find specific deficiencies in recording medical records for inpatient and outpatient care in health care facilities is to analyze their completeness. Based on interviews conducted by researchers at Bhayangkara Hospital Batam, the expected quality indicator must be achieved 100% but based on data in 2021 the incompleteness of the medical record file reached 17,75% and the completeness of the medical record file was only 82,25%. Therefore, the completeness of filling out the medical record file must reach 100% for 1x24 hours after the patient leaves the hospital. Objective: To find out the percentage of incomplete filling of quarterly outpatient medical record files at Bhayangkara Hospital Batam. Methods: The research method used in this study uses a quantitative descriptive method by conducting a document checklist based on the research objectives. Results: The percentage of incomplete patient identification on the medical record sheet at Bhayangkara Hospital Batam is 0%. The percentage of incompleteness in important reporting is 7,95%. The percentage of incomplete authentication is 8.8%. the percentage of incompleteness in the correct documentation is 2,75%. The percentage of incompleteness on the medical record sheet at Bhayangkara Hospital Batam is 5,1%. Conclusion : The incomplete filling of the medical record file is because the officers do not apply the existing SOPs besides that because the doctor is in a hurry to fill out the medical record file.
