Abstract
Asuhan Kebidanan Pada Ny. T Selama Masa Kehamilan, Persalinan, Nifas, Bayi Baru Lahir, dan Pelayanan Keluarga Berencana di PMB Veronika Sinaga Kota Batam Nurul Fajar Rina1 , Indah Mastikana2 , Sherly Mutiara3 1D-III Kebidanan, STIKes Awal Bros Batam, nurulfajarrina@gmail.com 2D-III Kebidanan, STIKes Awal Bros Batam, indahmst2@gmail.com 3D-III Kebidanan, STIKes Awal Bros Batam, sherly9391@gmail.com ABSTRAK Kehamilan dan persalinan termasuk hal yang fisiologis terjadi, namun jika tidak terpantau dengan baik, akan menimbulkan hal yang patologis. World Health Organization (WHO) menjelaskan pada tahun 2017 sekitar 295.000 wanita meninggal selama dan setelah kehamilan dan persalinan. AKI di Indonesia pada tahun 2017 sebanyak 1.712 kasus, AKB 24 per 1.000 kelahiran hidup. Salah satu upaya yang dapat dilakukan untuk menurunkan AKI dan AKB yaitu dengan perawatan terampil yang berkesinambungan sebelum, selama, dan setelah melahirkan. Tujuan dari penyusunan laporan tugas akhir ini adalah memberikan asuhan kebidanan secara berkesinambungan selama kehamilan, bersalin, nifas, bayi baru lahir, dan keluarga berencana pada Ny. T usia 25 tahun G2P1A1H0 di PMB Veronika Sinaga dengan pendekatan manajemen asuhan kebidanan dan mendokumentasikan dengan metode SOAP. Metode yang digunakan penulis yaitu jenis deskriptif, yang berupa penelitian dengan metode atau pendekatan study kasus, dengan kunjungan kehamilan dilakukan sebanyak 2 kali, ibu bersalin 1 kali, nifas 3 kali, bayi baru lahir 3 kali, dan konseling keluarga berencana 1 kali. Dari hasil pengkajian menunjukkan asuhan yang diberikan pada Ny T usia 25 tahun mulai dari kehamilan, bersalin, nifas, bayi baru lahir, dan pelayanan keluarga berencana berjalan lancar serta kondisi ibu dan bayi dalam keadaan normal. Kesimpulan pada asuhan kebidanan ini berdasarkan hasil pengkajian dan pemeriksaan telah diketahui dan dilakukan asuhan kebidanan berkesinambungan pada Ny. The World Health Organization (WHO) explains that in 2017 around 295,000 women died during and after pregnancy and childbirth. MMR in Indonesia in 2017 was 1,712 cases, IMR 24 per 1,000 live births. One of the efforts that can be done to reduce MMR and IMR is with continuous skilled care before, during, and after childbirth.The purpose of the preparation of this final report is to provide continuous midwifery care during pregnancy, childbirth, postpartum, newborn, and family planning to Ny. T 25 years old G2P1A1H0 at PMB Veronika Sinaga with a midwifery care management approach and documenting with the SOAP method. The method used by the author is descriptive type, in the form of research with a case study method or approach, with pregnancy visits carried out 2 times, mothers giving birth 1 time, postpartum 3 times, newborns 3 times, and family planning counseling 1 time. The results of the study showed that the care provided to Mrs. T at the age of 25 years starting from pregnancy, childbirth, postpartum, newborns, and family planning services was running smoothly and the condition of the mother and baby was in normal condition. The conclusion on this midwifery care based on the results of the assessment and examination has been known and carried out continuous midwifery care on Ny. T during pregnancy, childbirth, postpartum, newborn, and family planning services, and the authors can apply midwifery care through a midwifery management approach and documented by the SOAP method Keywords: Pregnancy, Childbirth, Postpartum, Newborn, Family Planning
