Abstract
Setiap tahun sekitar 810 wanita meninggal karena penyebab terkait kehamilan dan persalinan. Sebagian besar kehamilan ini berlangsung dengan aman, namun, sekitar 15% menderita komplikasi berat dengan sepertiganya merupakan komplikasi yang mengancam jiwa ibu. World Health Organization (WHO) di seluruh dunia, pada tahun 2017 AKI di dunia mencapai angka 295.000 jiwa. Di Indonesia AKI 305/100.000 kelahiran hidup dan AKB 21,12/1000 kelahiran hidup. Adapun tujuan dari penyusunan laporan tugas akhir ini untuk memberikan asuhan kebidanan secara berkesinambungan pada ibu hamil, bersalin, nifas, bayi baru lahir dan keluarga berencana dengan didokumentasikan menggunakan pendekatan manajemen SOAP. Metode yang digunakan penulis adalah wawancara, observasi, pemeriksaan fisik. Asuhan kebidanan secara berkesinambungan (Continuity Of Care) di PMB Atlantika, Amd.Keb diberikan pada Ny. E usia 37 tahun dengan kunjungan kehamilan dilakukan sebanyak 2 kali, ibu bersalin 1 kali, nifas 3 kali, neonatus 4 kali dan kontrasepsi 1 kali. Pada pemeriksaan kehamilan trimester III, ibu tidak ada keluhan. Proses persalinan Ny. E berlangsung normal dengan Kala I berlangsung selama 5 jam, Kala II 15 menit, bayi lahir pukul 07.16 WIB berjenis kelamin perempuan berat badan 3100 gram panjang badan 50 cm, Kala III 5 menit dan kala IV 2 jam. Pada asuhan masa nifas dan bayi baru lahir berlangsung secara fisiologis, ibu tidak mengalami keluhan atau masalah. Keadaan bayi dari kunjungan 1-4 dalam keadaan baik, tidak ada tanda-tanda infeksi dan kegawatdaruratan. Pada kunjungan kontrasepsi ibu diberikan konseling tentang kontrasepsi, manfaat kontrasepsi, kekurangan dan kelebihan kontrasepsi serta cara masing-masing penggunaan kontrasepsi. Kesimpulan pada asuhan kebidanan ini berdasarkan hasil dari pengkajian dan pemeriksaan telah diketahui dan dilakukan asuhan kebidanan berkesinambungan pada Ny. E dari masa hamil, persalinan, nifas, bayi baru lahir dan kontrasepsi serta penulis dapat menerapkan asuhan kebidanan melalui pendekatan manajemen kebidanan dan di dokumentasi dengan metode SOAP. Most of these pregnancies are safe, however, about 15% suffer from serious complications with one-third being life-threatening complications of the mother. World Health Organization (WHO) worldwide, in 2017 Maternal Mortality Rate (MMR) in the world reached 295,000 people. In Indonesia the MMR is 305 / 100,000 live births and the Infant Mortality Rate (IMR) 21.12 / 1000 live births. Therefore, the purpose of the final report’s was to provide a continuity of midwifery care during pregnancy, childbirth, postpartum, neonates and contraception documented by using SOAP management approach. The method used by the writer was interview, observation and physical examination. Continuity of care at PMB Atlantika, Amd.Keb was given to Mrs. E, age 37 years with 2 pregnancy visits, maternal mother 1 time, 3 childbirth, neonates 4 times and 1 time contraception. In the third trimester of pregnancy examination, the mother had no complaints. Mrs. E lasts normally with stage I lasts for 5 hours, stage II 15 minutes, the baby is born at 07.16 WIB, female sex, body weight 3100 grams, body length 50 cm, stage III 5 minutes and stage IV 2 hours. In the postpartum and newborn care physiologically, the mother does not experience any complaints or problems. Babies from visits 1-4 are in good condition, there are no signs of infection and emergency. During the contraceptive visit, the mother was given counseling about contraception, the benefits of contraception, the advantages and disadvantages of contraception and how to use contraception. The conclusion on this midwifery care based on the results of the assessment and examination has been known and carried out continuous midwifery care at Mrs. E from the period of pregnancy, childbirth, childbirth, newborns and contraception and the authors can apply midwifery care through a midwifery management approach and documented using the SOAP method.
