Abstract

PROGRAM STUDI DIII REKAM MEDIS DAN INFORMASI KESEHATAN UNIVERSITAS AWAL BROS Karya Tulis Ilmiah (KTI), 2022 ANALISIS KELENGKAPAN PENGISIAN BERKAS REKAM MEDIS RAWAT JALAN DI PUSKESMAS BALOI PERMAI BATAM TAHUN 2021 Uci Karmila, 00419015 X+ 79 Halaman + 2 Bagan + 8 Lampiran ABSTRAK Latar Belakang : Pengisian berkas Rekam Medis (RM) rawat jalan di puskesmas Baloi Permai dilakukan oleh dokter dan perawat dan harus kembali ke ruang PJRM 1x24 jam setelah pasien keluar Puskesmas. Apabila berkas RM belum lengkap, maka berkas akan dikembalikan ke poli pelayanan masing-masing untuk dilengkapi dalam kurun waktu 1x24 jam berikutnya. Berdasarkan observasi yang peneliti lakukan di Puskesmas Baloi Permai Batam indikator mutu yang diharapkan harus tercapai 100% namun berdasarkan data di tahun 2021 kelengkapan berkas rekam medis hanya mencapai 86,4%. Tujuan : Untuk mengetahui kelengkapan Rekam Medis di Puskesmas Baloi Permai Batam. Metode : Metode penelitian yang dipakai di penelitian ini memakai metode Deskriptif kuantitatif dengan melakukan checklist dokumen berdasarkan tujuan penelitian. Hasil : Presentase kelengkapan pengisian identifikasi pada lembar kartu rawat jalan di Puskesmas Baloi Permai pada Tahun 2021 yakni lengkap 100%. Presentase kelengkapan pengisian laporan yang penting terisi lengkap 92% . Yang tidak terisi lengkap 8%. Presentase kelengkapan pengisian autentifikasi lengkap 87,2%. Yang tidak terisi lengkap 12,8 % dan Pendokumentasian yang benar 100%. Jadi presentase kelengkapan pengisian lembar kartu rawat jalan di puskesmas baloi permai pada tahun 2021 yang terisi lengkap adalah 94,8% dan 5,2% tidak terisi lengkap. Kesimpulan : Ketidaklengkapan formulir rekam medis masih terdapat hambatan dalam unsur sumber daya manusia yang kurang, SOP yang tidak dijalankan, Petugas yang kurang teliti terhadap pengisian berkas rekam medis dan kurang optimalnya pelaksanaan ceklist kelengkapan formulir rekam medis. If the RM file is incomplete, then the file will be returned to the respective service poly for completion within the next 1x24 hours. Observation Based on researchers conducted at the Baloi Permai Health Center Batam, the expected quality indicator must be achieved 100% but based on data in 2021 the completeness of the medical record file only reached 86.4%. Objective: To find out the completeness of the Medical Record at the Baloi Permai Health Center Batam. Methods: The research method used in this study uses a quantitative descriptive method by conducting a document checklist based on the research objectives. Results: Presenting a knowing check on the outpatient card sheet at the Baloi Permai Health Center in 2021, which is 100% complete. The percentage of filling out important reports is 92% completely filled. 8% incomplete. The percentage of complete authentication is 87.2%. Incomplete 12.8% and 100% correct documentation. So the persenting of completeness of filling out the card sheet At the Baloi Permai Public Health Center in 2021, 94.8% and 5.2% were incompletely filled. Conclusion: Incomplete medical record forms still have obstacles in the element of lacking human resources, SOPs that do not work, officers who are less careful about filling out medical record files and less than optimal implementation of the checklist for completeness of medical record forms.
